It’s usually cancer or a nasty, lifelong digestive issue which can eventually lead to cancer. Throat, lungs, stomach or anything along the way gets damaged, scarred, hardens and rubs roughly against each other, making every breath and swallowing a neverending reminder that the rest of your short remaining timeline will be one dominated by pain and suffering and all you can do is wait for it to happen or just risk it all and bite a bullet now.
Or it’s just a heartburn. Probably ate something oily.
