I’ve only had experience with the last one: My cat would pee in the box but not poop. Fortunately she is very food motivated, so every time she used the litter box I would give her a treat. In days she was using the box every time no problem, after weeks of refusing to poop in it. So, maybe treats, or something else that he really likes? If that doesn’t work maybe try different kinds of litter, if you haven’t already.

For the first set of problems, I’m not well-versed in feline diseases, but it might be “rat ulcers” aka indolent ulcers, which are like sores on a cat’s mouth. He may not be eating much if they hurt. Internet says they might be treatable, depending on the cause… treating them might help solve the other problems.
